North Tooele Fire District Volunteer Application Instructions

Download and complete your application

  • Download the application to your computer before filling it out. Do not complete the version displayed on the webpage. Your information will not save correctly in the online version.
  • Open the application and look for the dropdown menu on the right side. Select Save as PDF or Download, depending on your browser.
  • Save the PDF somewhere you can find it, such as your Downloads folder or Desktop.
  • Open the saved PDF directly from that folder on your computer.
  • Select Save As and rename the file with your first and last name. Example: Jane Smith Volunteer Application.pdf.
  • Complete the application in the PDF saved on your computer, then save your changes.
  • Close and reopen the saved file to confirm that your answers appear.
  • Return to the North Tooele Fire District webpage. Click Choose File and select your completed, renamed application from your computer.
  • Remember: Download → Open from your computer → Fill out → Save with your name → Upload.

Required BCI background check

  • You MUST provide an official BCI Criminal History Report dated within the 90 days before NTFD reviews your application. Obtaining and submitting this report is the applicant’s responsibility.
  • A concealed weapons permit does not meet this requirement.
  • Visit the Utah Department of Public Safety, Bureau of Criminal Identification (BCI), to obtain your official report:

    • Phone: 801-965-4445
    • Hours: Monday–Friday, 8:00 a.m.–5:00 p.m., excluding state holidays
  • BCI currently states that an appointment is not required for a Utah Criminal History application. Bring a valid government-issued photo ID and payment for the report.
  • Scan the report or take clear photographs of every page. Make sure all information is readable and no edges are cut off.
  • If the images are on your phone, email them to yourself and download them to your computer.
  • Save the report with an easily identifiable name, such as Jane Smith BCI Report.pdf.
  • Upload the saved BCI report with your completed application. Keep the original report for your records. If submitting a paper application, attach the original BCI report to the back of the application.
  • Return to the North Tooele Fire District webpage. Click Choose File and select your BCI Report from your computer.
  • Applications submitted without the required BCI Criminal History Report will NOT be submitted for review.
